For Referring Professionals
At Endeavor House, we strive to make the referral and admissions process as easy as possible for referring professionals.
- Initial Referral: Call 732.264.3824 to speak with an intake coordinator. During this initial call, your client will be able to speak to the intake coordinator to ask any questions they may have about the Endeavor House programs. The intake coordinator will obtain client demographics and insurance information and ask that you fax pertinent medical records for review.(See Medical Records Required for more details)
- An intake Coordinator is available 24 hours a day to speak with any potential referral
- Admission appointments are booked as early as same-day
- Admissions can be processed in both Keyport and Kearny
- Admission Approval Process: Upon receipt of the medical records, the intake coordinator will consult with our medical and clinical professionals and confirm Endeavor House has the appropriate offerings to provide your client with the best treatment possible.
- Our expectation is to inform you of the status of your client’s admission within three hours of an initial call during regular business hours.
- Financial Counseling: After your client is approved for admission, the intake coordinator will call you back to relay the approval and obtain information on a financial contact to review benefit coverage and explain any out of pocket expenses (if applicable).
- Endeavor House encourages any family and support persons to discuss any questions about their loved one’s treatment program, insurance coverage or financial matters.
- If your client is being referred to us for continuing care, we may require the insurance pre-certification be completed prior to transfer to ensure medical necessity is verified.
- Transportation: Endeavor House provides transportation to and from local facilities for referrals once admission is confirmed and financial counseling is complete.
- Communication: Once your client is successfully admitted to our program, an intake Counselor will call you to inform you of the admission as well as pass on your contact information to the assigned clinician. Clinical staff will maintain regular communication with you based on your needs and involvement in the case.
