Outside Issues to be Addressed
Many of the individuals we serve enter treatment with unresolved outside issues. While these issues are important and need to be addressed during treatment, it is also essential that these outside issues not distract from the primary purpose of treatment. To follow are some common outside issues, as well as suggestions to help facilitate their resolution.
Legal Issues
A completed Release of Information form, signed by the client, is needed for any person or entity to be contacted by the client’s clinician. This may include attorneys, probation officers, and courts. Please provide full contact information, including name, address, and phone number, as well as fax number if possible.
Please provide two weeks notice of any upcoming court date, whenever possible. Clients are expected to postpone any court appearance while in inpatient treatment which may be done so with a letter from our facility.
Temporary Disability Paperwork
We encourage clients and their families to provide any state temporary disability paperwork at the time of admission. Temporary disability paperwork should be given to the client’s clinician. Note that not all states offer temporary disability benefits, and not all clients will qualify for benefits in their respective states of residence.
Medical Appointments
Endeavor House has a physician on-site to address client medical needs. In the event that an outside medical appointment is necessary, it must be coordinated through the facility nurse. Any questions regarding medications will be address by our medical staff.
